• requests directed towards EDPO Switzerland sàrl under the FADP with regard to Swiss data concerning persons in Switzerland; or

This form should therefore NOT be used for any requests that are not related to 1) EDPO Switzerland and 2) the FADP (e.g. customer service questions).

The personal data that you are asked to provide in this form is necessary for EDPO Switzerland to fulfill its obligations under Article 14 of the FADP as Data Protection Representative. The information that you provide will only be used for the purposes of identifying you and/or the person on whose behalf you are making the request and for handling your request.
