Swiss Federal Act on Data Protection (FADP) Data Request

You should only fill in this form if:

• You are a data protection authority OR an individual (i.e. data subject);
• You want to contact a company/organisation (i.e. one of EDPO Switzerland’s clients) in order to exercise your rights under the Swiss FADP

You should NOT fill in this form if:

• Your request is NOT related to the Swiss FADP (e.g. customer service questions);
• You are a company/organization/other legal entity;
• Your request does NOT concern an EDPO Switzerland client.

EDPO Switzerland can only handle requests directed to companies that have duly appointed EDPO Switzerland as its Swiss FADP Representative in Switzerland.

The personal data that you are asked to provide in this form is necessary for EDPO Switzerland Sàrl to fulfill its obligations under the Swiss FADP as Swiss Data Protection Representative. The information that you provide will only be used for the purposes of identifying you and/or the person on whose behalf you are making the request and for handling your request. The information that you provide in this form will be transferred to a country outside of Switzerland for purposes related to the handling of your request to exercise your rights under the Swiss FADP with respect to one of our clients. This country may not have equivalent data protection laws to Switzerland. By submitting your personal data, you are agreeing to this transfer and processing. We will take necessary steps to ensure that appropriate security measures are taken with the aim of ensuring that your privacy rights continue to be protected as outlined in our Privacy Policy.
